Five Ways Pharmacy is a community pharmacy at 192A Saint Vincent Street West, Ladywood, Birmingham, West Midlands, B16 8RP. We are registered with the General Pharmaceutical Council under premises registration 1119029. Five Ways Pharmacy is the data controller for the personal information you provide directly to us.

Patient details: where you contact or pay on behalf of someone else, the patient’s name and relevant details.
